Exam Form Clarification:
Exam Title – Exam 1, Midterm, Unit 1 Exam etc.
Test Length – This would be the time the regular class get, such as 50 minutes or 75 minutes.
Paper or Online?
Allowed Materials – Examples might be Calculator types, 3x5 notecard allowed, scratch paper, notes or data programs that are part of the exam.
Proctoring Instructions – Examples might include any passwords or special codes for online Exam, printing specifications, how students should submit exam or what proctors may need to check for. Anything you see fit that help us proctor your exams exactly as you would in class with no irregularities.

Upload your Exam – Exam itself, but other examples include Formula Sheets, Separate Instructions, Exhibits, Maps or any other supplemental materials. You can even upload, let’s say 3 different tests, and name them for a specific student should you want each student to receive a different variation.
Start and End Dates – Select the day of the Exam or set a range of days the Exam may be available for the student to complete. *If Date does not match or is not within the range selected by the student, exam will not generate. Or if you recently moved the date of your exam, make sure it is reflected in Accommodate too.
Always choose “Yes” to apply to all records -The wording may seem like you are giving away a copy of your exam materials to every student in the class…you are not! Only the student(s) who register, are approved and need it will have their accommodation records applied. Students NEVER have access to exams!
Save vs Submit – Save will keep your changes so you can continue editing, Submit will apply your changes when you finish.
